During or after, but NOT before, your designated registration period, if you encounter problems registering or have any special requests, please complete the form listed below for the appropriate semester below.  The form provides directions on the types of requests we can typically address. Requests on these forms are sent to the appropriate people.  Please note that due to circumstances beyond our control, we cannot necessarily fulfill all requests.
