
Documenting your assessment work
Every academic department and program at Queens College documents their assessment work. The deadline for the 2024-2025 Academic Year is May 16, 2025.
The purpose of assessment reporting is to:
- Clarify and scaffold assessment work.
- Focus on the change of outcomes over time.
- Align assessment activities with best practices.
Submit all your Assessment Reporting Documents to your Assessment Coordinator. These documents will also be attached to your department or program’s annual report to aid in writing Departmental Self-Study that occurs every seven years as part of Academic Program Review.
Assessment is one way in which Queens College participates in regular cycles of evaluation, assessment, and continuous improvement to fulfill expectations from our institutional accreditation.

Assessment Reporting Documents
Assessment Reporting Documents refer to Assessment Plans, Assessment Results Reports, and the Assessment Overview Survey. Templates for these documents are included above.
Assessment Plan
An Assessment Plan includes the program learning objective(s) you wish to assess, the courses in which the assessment will take place, appropriate assessment methods, and acceptable benchmarks for those methods.
Assessment Results Report
An Assessment Results Report is the documentation of the results from a recent Assessment Plan, and depending on whether the outcomes meet the desired benchmarks, includes either a Final Report or an Assessment Action Plan.
Final Report
A Final Report is the final summary of a completed assessment cycle.
Assessment Action Plan
An Assessment Action Plan discusses changes that will be made to the department or program in the coming year to improve student learning outcomes. Examples of changes include: curricular changes; development of materials, modules, courses; improving student support; building community connections.
Assessment Overview Survey
The Assessment Overview Survey is a yearly survey developed by the Academic Assessment Council to learn about the current state of assessment practice in the College. This document will help you self-assess your department or program’s progress towards best practices in assessment and provides feedback to the council about the types of professional development that would be most useful and appreciated.
